Why the scale doesn't move even though you eat little

It is the sentence I hear most often in consultations: "I eat almost nothing, and the scale won't move." And the first thing to say is: I believe you. This is not about secret snacking or weak character. Eating very little and not losing weight is a real, well-described state — and it usually means the body has switched into defence.
Defence mode: the metabolism adapts
The body reads a long, harsh calorie shortage as famine. It responds the way it was built to: it lowers its own spending. Temperature regulation, spontaneous movement, even thought feel more tired; the thyroid quietly turns the dial down. You eat 1,200 calories — and the body learns to run on 1,200. The deficit you created disappears from the inside.
Stress and sleep sit on the scale too
Chronic stress keeps cortisol high, and high cortisol does two unhelpful things at once: it encourages storage around the middle and it holds water. Short sleep shifts the hunger hormones within days. A woman who under-eats, over-worries and under-sleeps is fighting her own endocrine system — and the scale reports the system, not the salad.
When it's medical
Sometimes the block is physiological: an underactive thyroid, insulin resistance, certain medications. If you genuinely eat little, move regularly and the weight has not moved for months — that is a signal to test, not to cut further. A standard check-up read against your individual picture, not just the reference ranges, is where I always start.
The way out is more, not less
Counter-intuitively, the escape from defence mode is not a deeper cut. It is rebuilding trust: regular meals with enough protein, real food instead of 'diet' products, sleep treated as seriously as diet, movement that is daily rather than punishing. The metabolism recovers the way it slowed — gradually. In my practice this is exactly why nutrition is a lifestyle we build for a woman, never a temporary punishment: results that arrive organically are the ones that stay.
